Senior Backend Engineer

Responsibilities
- Architect subscription and billing infrastructure supporting medication switches, dynamic pharmacy pricing, payment recovery, and prorated billing
- Build reliable clinical ordering and fulfillment state machines integrating compounding pharmacies, controlled-substance compliance, and personalized dosing protocols
- Own AWS and Kubernetes infrastructure, CI/CD pipelines, observability, and safe deployment patterns
- Partner with Clinical, Operations, and Product teams to shape workflows and business outcomes
- Design backend architecture enabling AI-assisted clinical operations, patient safety improvements, and scalable provider workflows
- Make architectural decisions, modernize legacy systems, and raise engineering standards through code review, testing, and technical guidance
Requirements
- 5–7+ years of backend engineering experience operating complex production systems
- Experience with complex domain logic, distributed stateful workflows, and deep external-partner integrations
- Strong data modeling, schema design, state transition, and domain-boundary judgment
- Comfort with or willingness to master Kubernetes, Terraform, AWS, CI/CD, and system reliability ownership
- Experience using frontier agentic tools such as Codex 5.3 or Claude Opus 4.6 and auditing their output
- Excellent communication and collaboration in environments shaped by healthcare regulations, compliance, and patient safety
- Willingness to work in person five days per week in the San Francisco office
- Staff-level candidates should have a track record of setting technical direction across teams and leveling up engineers
